Registration
After the student has been accepted, the following items must be verified before registration:

Business Office clearance.

A signed "Consent for Treatment" form.

Signed statement that both student and parent or guardian have read and understood this bulletin.

Physical Examination Report (Form 14).

Social Security Number.

Placement Test / Entrance Exam: TOEFL Test (for International Students).

For school record identification, employment purposes and ease of completing many reports and forms which require Social Security numbers, students are required to obtain a Social Security Number before the school year begins. No students may work on campus without a Social Security Number.
Forms to apply for a Social Security Number are available at the Social Security office in the Honolulu Federal Building.
